Forms may require student, faculty, or staff signatures. Please review the following documentation on Creating Your Digital Signature (PDF, 358KB) to create your digital signature needed to complete each form. Once created you will be able to save for future signatures in Adobe PDF. In order to use your digital signature, you will need to download the form and open in Adobe, you cannot sign the form if you are filling it out in your browser. In addition to your digital signature, all forms will need to be sent to the Office of the Registrar at firstname.lastname@example.org from a UWG email account.
